STOP-IMH 2026 explores whether aspirin can be withdrawn immediately after primary PCI in patients with STEMI. In this pilot randomized trial of 200 patients, ticagrelor monotherapy showed similar observed MACCE rates compared with ticagrelor plus aspirin, while non-access-site bleeding was significantly lower. The findings provide a hypothesis-generating signal supporting further investigation of aspirin-free strategies immediately after PCI, while emphasizing that current guideline-recommended antiplatelet strategies and individual ischemic and bleeding risks should continue to guide clinical practice.
